Blackheads: most of what you are squeezing out of your nose is normal
By the team at New Age Spa · Updated 1 September 2026
The small greyish dots covering the nose in an even pattern are usually sebaceous filaments — ordinary pore anatomy, present on almost everyone, and they refill within days no matter what you do. A great deal of frustration and money goes into trying to permanently remove something that is not a flaw.
A real blackhead is not dirt. It is trapped oil and dead skin whose surface has darkened on contact with air. That is why scrubbing harder never works, and why the honest treatment is slow: you have to change what the pore is doing, not clean it out.
Telling them apart
| What you are seeing | How it looks | What it needs |
|---|---|---|
| Sebaceous filaments | Uniform grey or tan dots, evenly spread across the nose and inner cheeks, flat to the touch | Nothing. They are normal, and they return in days |
| Blackhead (open comedo) | A distinctly dark plug in a single visibly widened pore, scattered rather than uniform | Consistent topical work over weeks; extraction only clears what is there now |
| Whitehead (closed comedo) | A small firm skin-coloured bump under the surface, no opening | The same topical work. Squeezing these is how marks are made |
| Inflamed spots | Red, sore, sometimes with pus | Acne rather than congestion — see acne treatment |
| Milia | Hard white beads, often around the eyes, that never come out with pressure | Professional removal. Pressure alone will not shift them |
Why scrubbing makes it worse
Congestion feels like something that should come off, so the instinct is to strip: harsh cleansers, grainy scrubs, astringent toners, a stack of acids. Skin that is stripped repeatedly responds by producing more oil and by getting irritated, and irritated skin is more prone to blocked pores, not less. It is one of the more reliable ways to spend six months making a mild problem into a stubborn one.
If you are currently escalating, the ceiling is described in exfoliation: how much is too much. The oil side of the same argument is in oily skin.
Pore strips, and what they actually do
A pore strip pulls out the top of whatever is sitting in the pore — mostly filaments, some plugs. The pores look emptier for a day or two and then look exactly as they did. The strip has done nothing to the pore lining or to the oil production feeding it, and repeated aggressive use can irritate the surrounding skin. It is a cosmetic reset, not a treatment, and it is fine to use it as one as long as you know that is what you bought.
What genuinely changes congestion
- A gentle cleanser used twice a day. Unremarkable, and the part most people skip past on the way to something stronger.
- One well-chosen active, used consistently, rather than four rotated impatiently. Ingredients that work inside the pore and ingredients that normalise how the pore lining sheds are the two useful families — a pharmacist or clinician can point you to a starting product.
- Weeks, not days. A pore that is currently blocked will clear on its own timeline. What consistent treatment does is stop the next one forming.
- Non-comedogenic products, including in make-up and sunscreen, since a heavy occlusive layer over congested skin undoes the rest.
- Leaving whiteheads alone. A closed comedo has no opening. Forcing one open injures the skin around it, and that is where the brown mark or small scar comes from.
Where extractions fit
Professional extractions clear what is in the pore right now, on softened skin, with even pressure and clean technique. They are satisfying, they make skin look immediately better, and they do not change what happens next month — that is the topical routine's job. We will extract what comes out reasonably and stop there. Anything that resists is left alone rather than forced, because forcing it is how a two-week mark is created to fix a two-day blemish.
If you already have marks from past squeezing, the sorting between a mark and an actual scar is in most acne scars are not scars. Widened pores as a standing complaint are covered in enlarged pores.
What we will not do
- Promise to remove sebaceous filaments permanently. Nobody can, and any clinic saying otherwise is selling you a repeat visit.
- Force an extraction on skin that is inflamed or on a lesion that will not release.
- Stack aggressive exfoliation onto skin that is already irritated.
- Treat widespread inflamed acne as a congestion problem. That belongs with a physician.
See a doctor if
Spots are painful, deep, cystic or leaving marks, if congestion is widespread across the back and chest as well as the face, if it appeared suddenly alongside other changes, or if several months of sensible over-the-counter treatment have not helped. Effective prescription treatment exists and there is no reason to spend a year without it.
